Palo Alto County Community Health Services
If you are recovering from an illness or injury, or just need help with personal care needs to remain independent in your own home, Community Health Services could provide you with the care you need. Our state certified public health nurses work with your physician to continue treatment started in the hospital or nursing home and provide you with Medicare certified in-home nursing care.
Home Health Services:
- Medicare certified in-home Nursing Care
- Home infusion therapy
- Patient Education
- Physicial, Occupational and Speech Therapy services
Maternal/Child Care Services:
- Immunization clinics
- Home visits to new moms
- Pre-natal-Childbirth classes
Health Promotions:
- Stop smoking classes
- Communicable disease prevention
- Adult immunizations
- Family and individual health education
Home Care Aide/ Homemaker Services:
- Assistance in personal care needs
- Light housekeeping
- Some shopping needs
- Installation of Lifeline units
- Care for terminally ill patient and family
- Respite care for caregivers
Referrals:
Referrals to Community Health Services can be made by anyone who
sees a health need.
